Dehydroepiandrosterone (DHEA) is a sex hormone precursor (something needed to make hormones like testosterone) released by the testes and adrenal glands. Like testosterone, DHEA production in men peaks around age 30 before it starts declining. In addition to boosting testosterone levels, DHEA also increases nitric oxide in the bloodstream. A large study involving 17 hormones given to men over 55 concluded that only DHEA increased sexual performance in subjects with erectile dysfunction.8

Experts feel that treating erectile dysfunction on your own, without consulting a doctor, is unsafe. "If you have ED, the first thing you need is a diagnosis," says impotence expert Steven Lamm, MD, a New York City internist and the author of The Hardness Factor (Harper Collins) and other books on male sexual health. He says men with severe erectile dysfunction probably need one of the prescription ED drugs, which include the PDE-5 inhibitors Levitra (vardenafil), Cialis (tadalafil), and Stendra (avanafil), as well as Viagra. But, he says, mild ED -- including the feeling that "you're not as hard as you could be" -- often responds to natural remedies.
Three patients had a severe fall in blood pressure 1 hour after sildenafil (one at visit 2 and two at visit 5), the standing blood pressure falling from 128/85 to 65/55, 104/60 to 56/32, and 115/70 to 55/39 respectively. All three remained conscious but had severe symptoms and signs of orthostatic hypotension, feeling unwell and unable to stand for more than a few seconds. Each patient had to wait (for 4 hours in one patient) until the drop in blood pressure became tolerable. Figure 1 shows the average mean blood pressure changes in this group. On the basis of this adverse event a decision was taken to unblind the study and discontinue recruitment.
The extensive clinical database that already exists for sildenafil citrate demonstrates that it is efficacious in the management of erectile dysfunction, and safe when appropriately used. The contraindication with any form of nitrate therapy is absolute because a synergistic response can result in severe hypotension.9 In normal volunteer studies sildenafil reduced mean maximum supine systolic and diastolic blood pressure by about 8 mm Hg and 5 mm Hg without an effect on the heart rate.
