Consider a starting dose of 25 mg in patients > 65 years, patients with hepatic impairment (e.g., cirrhosis), and patients with severe renal impairment (creatinine clearance < 30 mL/minute) because administration of VIAGRA in these patients resulted in higher plasma levels of sildenafil [see Use in Specific Populations and CLINICAL PHARMACOLOGY].

Some men report an improvement in ED symptoms after having acupuncture. This ancient form of medicine involves using tiny needles to stimulate certain points on the body. While study results are mixed on whether acupuncture really works for ED, the therapy is considered safe and may help you feel better overall.

Herbal supplements such as ginkgo biloba, saw palmetto, and yohimbe have been touted as sexual enhancers, and some men have been tempted to try them to treat erectile dysfunction. Bennett warns, however, that none has been approved by the FDA or even shown by any reliable studies to prevent, treat, or improve erectile dysfunction. Moreover, supplements are unregulated and can have many side effects or interfere with prescribed medications you’re already taking. Don’t jeopardize your health by taking a supplement to treat erectile dysfunction without first talking with your doctor.
There are other similar medicines called tadalafil (Cialis), vardenafil (Levitra) and avanafil (Spedra) that work in a similar way. Buying Viagra online
Ginkgo biloba. Known primarily as an herbal treatment for cognitive decline, ginkgo has also been used to treat erectile dysfunction -- especially cases caused by the use of certain antidepressant medications. But the evidence isn't very convincing. One 1998 study published in the Journal of Sex & Marital Therapy found that it did work. But a more rigorous study, published in Human Pharmacology in 2002, failed to replicate this finding. "Ginkgo has come out of fashion in the past few years," says Ronald Tamler, MD, assistant professor of medicine and codirector of the men's health program at Mount Sinai Medical Center in New York City. "That's because it doesn't do much. I can say that in my practice, I have not seen ginkgo work -- ever."
But do any of these things really work? Eric Laborde, MD, a spokesman for the American Urological Association, is skeptical.

PDE-5 inhibitors work by blocking the activity of PDE-5, an enzyme that plays a role in the contraction of smooth muscles around blood vessels. PDE-5 inhibitors prevent the PDE-5 enzyme from stimulating PDE-5 receptors, a type of protein molecule that is abundant in the smooth muscles of pulmonary blood vessels and the penis.
Other research also supports eating more plants. In one study, about one-third of men who cut back on their consumption of cholesterol and saturated fat (both of which are found mainly in animal products) and increased their intake of fiber (found in plants) recovered their normal sexual function.
On the horizon is gene therapy that would deliver genes that produce products or proteins that may not be functioning properly in the penile tissue of men with ED. Replacement of these proteins may result in improvement in erectile function. Experimental animal models have demonstrated improvement in erectile function with gene therapy. Human studies may also demonstrate success with this therapy. Gene therapy may take a long time for regulatory approval and public acceptance.
