No dose adjustment is required for mild (CLcr=50-80 mL/min) and moderate (CLcr=30-49 mL/min) renal impairment. In volunteers with severe renal impairment (Clcr < 30 mL/min), sildenafil clearance was reduced, resulting in higher plasma exposure of sildenafil (~2 fold), approximately doubling of Cmax and AUC. A starting dose of 25 mg should be considered in patients with severe renal impairment [see DOSAGE AND ADMINISTRATION and CLINICAL PHARMACOLOGY].
With long-term treatment options, the symptoms of ED can effectively be reversed for years in most men.

Several herbs and dietary supplements claim to improve erectile dysfunction (ED) and libido, with some even claiming to work like Viagra. However, the U.S. Food and Drug Administration (FDA) does not approve of any “herb” or “supplement” meant to improve sexual stamina, so consult your doctor before ingesting these potentially dangerous drugs. Antioxidants in berries help nitric oxide last longer in the bloodstream, which is where the pomegranate myth began. However, there is no scientific proof that drinking this antioxidant-rich juice can actually help with erections. “Of course, it’s better to drink a glass of pomegranate juice than a soda, but it won’t help” with ED, Nelson says.
Compared to other erectile dysfunction treatment pills, the GAINSWave procedure is revolutionary. The therapy is the quickest, safest, and most effective treatment available and does not require any chemicals or erectile dysfunction surgery.

“It is hard to know how many men are using ED drugs recreationally,” said DeSouza. "Studies are hard to do because they rely on self-reporting. Men who want to use these drugs can often get them by answering questions about erectile dysfunction appropriately when they talk to a doctor. They know the symptoms of ED."
Sexual activity can put an extra strain on your heart, especially if your heart is already weak from a heart attack or heart disease. Ask your doctor if your heart is healthy enough to handle the extra strain of having sex.
For many years, we have used medication to improve blood vessel circulation in the penis. These medications include papaverine, phentolamine, and prostaglandin E1 — Trimix is a mixture of all three of these drugs. Scientists have found that these drugs can induce a penile erection when injected directly into the penis.
In addition, N-desmethyl metabolite AUC and Cmax values significantly increased by 200% and 79%, respectively in subjects with severe renal impairment compared to subjects with normal renal function.
However, previous studies have shown that erectile dysfunction is more common in treated, than untreated, men with high blood pressure. And certain antihypertensive drugs – notably diuretics and beta blockers – have been linked with deterioration in sexual function. This study examined the association between blood pressure level and penile blood flow, and whether blood pressure-lowering medication had an effect on the relationship.
Obesity, nutrient deficiencies and eating inflammatory foods can also increase the odds of impotence. Of the natural remedies for impotence, this is one that can also help boost your health in other important ways. Here are foods that can help reverse some of these problems and promote better circulation, heart health and even mental well-being: High fiber diet — High-fiber foods like nuts, seeds, fruit, and vegetables support hormones and detoxification which can improve impotence. Foods high in vitamin E — A diet rich in vitamin E, such as wheat germ and green leafy vegetables, will help improve blood flow. Foods high in zinc — Pumpkin seeds, sunflower seeds, chia seeds, beef, lamb, and spinach are good sources of zinc that help improve testosterone. Nuts & seeds — Watercress and sesame seeds — Both can improve libido, so try to consume some every day. Brazil nuts — These nuts are high in selenium, which plays a role in maintaining healthy testosterone levels.
Transitory pain — About 10 percent of men develop pain or discomfort in the head of their penis shortly after injection. This subsides spontaneously after a few minutes and is more common in men with impotence secondary to nerve problems.
Finally, Dr Montorsi et al found that severe ED was more common in patients with multivessel coronary involvement as compared with those with single-vessel disease (31% vs 12.5%; p<0.01). They also found a significant inverse relationship between the extent of CVD and IIEF scores.31
Several medications are not recommended for the treatment of erectile dysfunction. These include trazadone, yohimbine, and herbal therapies, as there is no evidence to support their safe use for the treatment of erectile dysfunction. It is important to note that testosterone therapy is not indicated for the treatment of erectile dysfunction in the patient with a normal serum testosterone level.
