And, by the way, an erection that lasts four hours is not a good thing. It is painful and dangerous.

Though erections may seem straightforward, the science behind them is complicated. At a basic level, erections happen thanks to a balance of blood flow into and out of the penis. When vascular, hormonal, nerve, or other physical or psychological changes disrupt blood flow, it’s harder to get and keep erections.
Overall the baseline and 10 week postplacebo data for IIEF (questions 3 and 4) and the QoL for whole life and sex life were similar. The same responses after 10 weeks of active medication starting with 50 mg and titrated up to 100 mg in most patients showed a marked and significant improvement in being able to obtain (question 3) and maintain an erection (question 4) for sexual intercourse. Although there was little change in the whole life QoL there was a marked improvement in sex life (table 1). The partner questionnaire response confirmed the improvement. There were no serious adverse events over the study period. One patient reported headache and a warm feeling after taking the active medication but did not discontinue it.
In the current study, most of the participants reported that they got information about sildenafil primarily from their friends. Similar results were reported in other studies in which friends, the internet, and sex shops were the primary source of information (6). Information sought from sources other than health professionals could lead to unexpected consequences that could be directly related to the medication (sildenafil) itself, such as priapism or due to an interaction between sildenafil and other medications the user may take or comorbid conditions he/she may have (4, 5, 13–18). The recreational use of sildenafil was also associated with vision changes (24).

The results from this pilot study are shown in Table 3; the mean resting systolic and diastolic blood pressures decreased by 7% and 10% compared to baseline in these patients. Mean resting values for right atrial pressure, pulmonary artery pressure, pulmonary artery occluded pressure and cardiac output decreased by 28%, 28%, 20% and 7% respectively. Even though this total dosage produced plasma sildenafil concentrations which were approximately 2 to 5 times higher than the mean maximum plasma concentrations following a single oral dose of 100 mg in healthy male volunteers, the hemodynamic response to exercise was preserved in these patients.
